The Transitional Care Service can provide support for up to 28 days after discharge from hospital. The Transitional Care Team will make an appointment to see consumers within 7 days of discharge at either the Caboolture Hospital or Redcliffe Adult Mental Health Service. It\u2019s also possible to negotiate for appointments to be at home or another community agency.
