My Health Now<\/h1><\/div>

\"\"<\/a>My Health Now<\/em> is an easy-to-use form that helps you have your important health information in your hands when you need it most.<\/p>\n

Based on an idea by Jurina (pictured below) and developed with the Caring Together Consumer Network<\/a>, My Health Now<\/em> takes the stress out of remembering your health history and key contacts when you\u2019re unwell.<\/p>\n

It has the basic information you will be asked for in an Emergency Department or outpatient appointment, as well as other things you may want your health care team to know.<\/p>\n

How to use My Health Now<\/em>:<\/p>\n

  1. Download a word version to your computer.<\/a><\/li>\n
  2. Fill it in with your information.<\/li>\n
  3. Add what is important to you.<\/li>\n
  4. Save your form to your phone or computer.<\/li>\n
  5. Print copies for you and your loved ones.<\/li>\n
  6. Keep copies where you can get them in a hurry if you need e.g. handbag, car, fridge.<\/li>\n
  7. Give to ambulance staff, hospital staff, or your care team when you need to.<\/li>\n<\/ol>\n<\/div>\n<\/div>

    <\/i><\/i><\/span>Why should I use the My Health Now form?<\/span><\/a><\/h4><\/div>

    Going to hospital can be sudden and stressful. You may be injured or unwell and you will be asked many questions about you and your health. We know it can be hard to remember everything you need to share. My Health Now<\/em> ensures you have your information in your hands when you need it.<\/p>\n<\/div><\/div><\/div>

    <\/i><\/i><\/span>Can I update or change it?<\/span><\/a><\/h4><\/div>

    Yes. You should update it when you need to.<\/p>\n

    <\/i><\/i><\/span>Is this a legal document?<\/span><\/a><\/h4><\/div>

    No. My Health Now<\/em> is designed to help you to remember and communicate your health information with ambulance or hospital staff if you are unwell. It is not an official health document and is not legally binding. <\/strong><\/p>\n<\/div><\/div><\/div>

    <\/i><\/i><\/span>My health is very complex. Is My Health Now for me?<\/span><\/a><\/h4><\/div>

    While My Health Now<\/em> will be useful for many people, if your health is very complex there are other options that may work better for you, such as Julian\u2019s Key<\/a>.<\/p>\n

    <\/i><\/i><\/span>Can I fill in a My Health Now form for my kids or someone I care for?<\/span><\/a><\/h4><\/div>

    Yes. You can make copies of My Health Now<\/em> for anyone you care for.<\/p>\n<\/div><\/div><\/div>

    <\/i><\/i><\/span>Does this replace My Health Record?<\/span><\/a><\/h4><\/div>

    No. My Health Record<\/a> is a comprehensive digital record of your health information and experiences. While you can update your basic information, My Health Record includes Medicare and clinical documents uploaded by health professionals. My Health Record is very important for your health care team to be able to see your previous health experiences, such as test results and x-rays.<\/p>\n

    My Health Now<\/em> is your summary of your health history that lets you to share what\u2019s important to you now.<\/p>\n

    Name:<\/strong> Jurina<\/p>\n

    A little about Jurina<\/strong><\/p>\n

    I was diagnosed with breast cancer soon after moving to Queensland and had no friends locally.\u00a0I attended a Pink Lady event in Brisbane and later became a Health Consumer Advocate with Breast Cancer Network Australia.\u00a0A breast cancer support group started up at Caboolture Hospital in 2000 and I heard about problems people were having, particularly with lack of Reconstructive Surgery and Lymphoedema Services.\u00a0That kick-started my advocacy activities in Queensland. I later joined Health Consumers Queensland, Metro North Health and in particular the Caboolture Hospital Caring Together Consumer Network as soon as I heard about it nearly 7 years ago.\u00a0<\/p>\n

    Where did the idea for My Health Now come from? \u00a0<\/strong><\/p>\n

    I recently attended a hospital appointment and was given a form to fill in.\u00a0I found it very difficult to recall parts of my health history and impossible to recall the dates.\u00a0When I got home, I sat down and created a My Health History document that I could easily edit as needed and could take with me to all my health-related appointments.\u00a0I shared the template with our Caboolture Hospital Caring Together Consumer Network and we all agreed that it is a valuable resource that could benefit everyone in our community.<\/p>\n

    What is one thing about the health care system you wish everyone knew?\u00a0 <\/strong><\/p>\n

    We all appreciate our fantastic health care system but many of us recognise areas that could be improved on, yet we don\u2019t want to be seen to be complaining so we keep quiet. If we don\u2019t suggest improvements, then no one knows that things could improve.\u00a0I wish everyone could feel safe and confident about speaking up and suggesting changes that could benefit patients and staff alike.<\/p>\n<\/div><\/div><\/div>
