People who will benefit from this service are those who have had a recent injury, illness or surgery (within the past six months) which has resulted in rapid functional decline and has impacted on their ability to complete everyday activities.<\/p>\n","conditions_treated":"","our_services":"<p>Our services and programs, including home visits (if required), run for 4, 8 and 12 weeks and include:<\/p>\n<ul>\n<li>Physiotherapy<\/li>\n<li>Occupational Therapy<\/li>\n<li>Speech Pathology<\/li>\n<li>Social Work<\/li>\n<li>Leisure Therapy<\/li>\n<li>Rehabilitation Consultancy.<\/li>\n<\/ul>\n<p>We provide rehabilitation for people who are recovering from or have been diagnosed with:<\/p>\n<ul>\n<li>stroke<\/li>\n<li>acquired brain injury<\/li>\n<li>early stage or acute exacerbation of neurodegenerative conditions with rehabilitation capacity<\/li>\n<li>an orthopaedic condition<\/li>\n<li>lower limb amputation to provide prosthetic rehabilitation<\/li>\n<\/ul>\n","include_link_to_healthcare_professionals_section":false,"gd_other_content":false}],"accessing_this_service":[{"variation":"freetext","free_text":"<p>To access this service, your GP or medical practitioner will need to send a referral letter.<\/p>\n<p>If there is a waiting list, you will receive a confirmation letter and be advised on what to do next. You will then undergo assessments and therapy based on your concerns and goals with our team members.<\/p>\n<p>You will be expected to attend your appointments throughout your block of rehabilitation. This may be in the clinic, via Telehealth, or with a visit to your house. If you are unable to attend, it is expected that you call us to let us know.<\/p>\n<h3>Length of program<\/h3>\n<p>The blocks of therapy are usually 4, 8 or 12 weeks long, depending on your needs. This will be discussed at your comprehensive assessment.<\/p>\n<h3>How much does it cost?<\/h3>\n<p>CBRT is a completely subsidised program \u2013 there is no cost involved.<\/p>\n"}]}]}],"general_content_2":false,"clinic_introduction":"","clinic_details":[{"clinic":""}],"general_content_3":false,"frequently_asked_questions":false,"healthcare_professionals":[{"for_healthcare_professionals":"<h3>Inclusion criteria<\/h3>\n<ul>\n<li>Consent to receive service, and must have both the desire and ability to participate in a self-directed therapy program (or use carer support), under guidance of the CBRT team<\/li>\n<li>Be over 18 years of age and must live (or temporarily be staying) in the geographical catchment area of Metro North Hospital and Health Service<\/li>\n<li>Have a recent health event (past 6 months) with functional change impacting on ability to participate in life roles<\/li>\n<li>Have functional rehabilitation goals that are achievable with time limited community-based therapy, with a focus on improving independence at home and\/or in the community<\/li>\n<li>Clinical judgement will be applied to all referred clients on a case by case basis<\/li>\n<\/ul>\n<h3>Exclusion criteria<\/h3>\n<p>Not appropriate for CBRT<\/p>\n<ul>\n<li>Immediate risk of hospital admission, ED presentation or harm due to medical instability, acute mental health conditions, or psychosocial reasons (eg; current living situation unsafe or no fixed abode, inadequate care support at home)<\/li>\n<li>No functional rehabilitation goals identified<\/li>\n<li>Requiring only a single discipline \u2013 including only requiring medical review.\n<ul>\n<li><strong><em>Note<\/em><\/strong><em>: If the client requires single discipline rehabilitation, a discussion with the Allied Health Team Leader is required prior to the referral, regarding how the service may be able to accommodate the rehabilitation needs of the client or if another service would be more appropriate.<\/em><\/li>\n<\/ul>\n<\/li>\n<li>Cancer clients who are unable to participate in rehab due to ongoing active medical treatment<\/li>\n<li>Non or partial weight bearing, awaiting an upgrade to full weight bearing status and\/or requires a hoist for transfers.<\/li>\n<li>Active and untreated substance abuse which inhibits ability to participate and achieve therapy goals<\/li>\n<li>Deconditioning in the absence of an acute medical event or hospitalization<\/li>\n<li>Functional symptoms without a confirmed diagnosis (e.g., FND)<\/li>\n<li>Review of existing or set up of new equipment that is essential for discharge from inpatient environment<\/li>\n<li>Carer training only required<\/li>\n<li>Function not adequate for basic needs \u2013 e.g., needing 1A to mobilize but limited access to carer support, no functional communication<\/li>\n<li>Likely to improve current mobility or function without intervention<\/li>\n<li>Previous rehabilitation with unsuccessful or limited gains<\/li>\n<li>Client (or NOK) has not consented to referral, or the client does not wish to engage<\/li>\n<li>Living environment deemed unsafe for CBRT staff to visit (eg;
