Community Based Rehabilitation Team (CBRT) aims to improve people\u2019s well-being, independence and quality of life. People who will benefit from this service are those who have had a recent injury, illness or surgery (within the past six months) which has resulted in rapid functional decline and has impacted on their ability to complete everyday activities.<\/p>\n","conditions_treated":"","our_services":"
Our services and programs, including home visits (if required), run for 4, 8 and 12 weeks and include:<\/p>\n
We provide rehabilitation for people who are recovering from or have been diagnosed with:<\/p>\n
To access this service, your GP or medical practitioner will need to send a referral letter.<\/p>\n
If there is a waiting list, you will receive a confirmation letter and be advised on what to do next. If there is no waiting list we will contact you to arrange a suitable time for your appointment.<\/p>\n

You will then undergo assessments and therapy based on your concerns and goals with our team members.<\/p>\n You will be expected to attend your appointments throughout your block of rehabilitation. This may be in the clinic, via Telehealth, or with a visit to your house. If you are unable to attend, it is expected that you call us to let us know.<\/p>\n The blocks of therapy are usually 4, 8 or 12 weeks long, depending on your needs.
