You will still be considered an inpatient of the hospital while on the GEMITH program.\u00a0<\/p>\n<p>The aim of the service is to care at home instead of in hospital or a health clinic.\u00a0<\/p>\n","conditions_treated":"","our_services":"<p>The team comprises of the following staff:<\/p>\n<ul>\n<li><strong>Geriatrician:<\/strong>\n<ul>\n<li>Specialist doctor who provides care for older people.<\/li>\n<li>Provide oversight and management of your care under the service.<\/li>\n<li>Can request tests and scans as required.<\/li>\n<li>Liaise with your usual care team.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Nurses:<\/strong>\n<ul>\n<li>Monitor health conditions following transfer from hospital.<\/li>\n<li>Manage intravenous medications and injections and wound dressings as directed by the Doctor .<\/li>\n<li>Provides education on health issues as necessary.<\/li>\n<li>Assess patient\u2019s suitability for hygiene assistance if needs indicate this is required .<\/li>\n<\/ul>\n<\/li>\n<li><strong>Occupational Therapist:<\/strong>\n<ul>\n<li>Review how you manage in your home environment.<\/li>\n<li>Advise on home safety including bathroom and toilet safety\/equipment.<\/li>\n<li>Organise equipment loan (eg. shower chairs) for a trial period.<\/li>\n<li>Discuss different ways to maintain your independence at home.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Physiotherapist:<\/strong>\n<ul>\n<li>Comprehensive assessment of your mobility, walking, equipment safety, and risk of falls.<\/li>\n<li>Physical rehabilitation through customised exercise program focussing on your strength, balance,<br \/>\noptimising mobility and falls prevention.<\/li>\n<li>Loan appropriate mobility equipment (eg. wheelie walker) for a trial period.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Clinical Assistant:<\/strong>\n<ul>\n<li>Assist with delivering and setting up loaned equipment at home.<\/li>\n<li>Supervise home exercise programs.<\/li>\n<li>Support activities prescribed by other staff members.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Social Worker:<\/strong>\n<ul>\n<li>Provide information on government and non\u2013government services available in the community (eg. Centrelink, ACAT and Carer Support services).<\/li>\n<li>Discuss legal documentation eg. Enduring Power of Attorney &amp; Wills.<\/li>\n<li>Provide counselling and support to patients and their families as needed.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Pharmacist:<\/strong>\n<ul>\n<li>Review your medications and dispense any new medications if required.<\/li>\n<li>Medication education and counselling.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Speech Pathologist:<\/strong>\n<ul>\n<li>Assess and manage swallowing difficulties<\/li>\n<li>Assess and manage communication difficulties<\/li>\n<li>Provide education and support to patients and their families<\/li>\n<\/ul>\n<\/li>\n<li><strong>Dietitian:<\/strong>\n<ul>\n<li>Assessment, education and intervention on nutritional requirements<\/li>\n<li>Education and intervention for those with or at risk of malnutrition<\/li>\n<li>Nutrition supplement advice<\/li>\n<\/ul>\n<\/li>\n<li><strong>Administration Officer:<\/strong>\n<ul>\n<li>Available Monday to Friday 8.00am to 4.30pm on 3049 1414<\/li>\n<li>Be available to take your phone calls and messages for our staff members as required<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n","include_link_to_healthcare_professionals_section":false,"gd_other_content":[{"general_content":[{"heading":"","content":""}]}]}],"accessing_this_service":[{"variation":"freetext","free_text":"<p>You will require a medical referral to access this service.
