A specialised multidisciplinary team works with patients carrying out comprehensive assessment, formulating patient-centred goals, treatment and discharge plans.<\/p>\n<p>The focus of GEMS is to:<\/p>\n<ul>\n<li>promote independence<\/li>\n<li>facilitate older people in improving functional capacity<\/li>\n<li>prevent further decline<\/li>\n<li>prepare patients to return to their home environments if suitable.<\/li>\n<\/ul>\n<p><strong>GEMS patients have access to a range of services including:<\/strong><\/p>\n<ul>\n<li>cognitive and functional assessment<\/li>\n<li>restorative care assessment<\/li>\n<li>geriatric assessment<\/li>\n<li>geriatric rehabilitation<\/li>\n<li>dementia assessment and management<\/li>\n<li>home visits to facilitate discharge planning.<\/li>\n<\/ul>\n","include_link_to_healthcare_professionals_section":false,"gd_other_content":[{"general_content":[{"heading":"","content":""}]}]}],"accessing_this_service":[{"variation":"freetext","free_text":"<p>Referrals are received from health professionals from the acute patient\u2019s care team or in public hospital by the Geriatric Rehabilitation and Liaison Service. Family members are asked to launder your clothing.<\/li>\n<li>Safe, comfortable, appropriate, and well-fitting footwear<\/li>\n<li>Night attire and toiletries (soap, shampoo, conditioner, deodorant, tissues, shaving kits, creams\/lotions).<\/li>\n<li>It does get cold sometimes in the unit, it is recommended that you also bring some warm clothes.<\/li>\n<li>Any other items you usually use at home \u2013 walking aids, wheelchairs, pressure relieving cushions, splints, pressure garments, glasses, hearing aids, dentures.<\/li>\n<li>Free WiFi is available. NB. You\/your family are responsible for any valuable items you choose to bring.<\/li>\n<li>Please bring all your current medications, including vitamin supplements and herbal medicines so your doctor and pharmacist can review them.<\/li>\n<\/ul>\n"}],"what_to_bring_mental_health":false,"what_to_bring_child_health":false}],"general_content":[{"other_content":[{"heading":"","content":"<h2>Visiting<\/h2>\n<p>Visiting hours are flexible, and vary from ward to ward. Some wards\/areas have restricted visiting hours due to the nature of care being provided in that particular clinical area and most wards have rest periods. Visitors can ask ward staff about the particular visiting arrangements for the ward where their relative or friend is staying.<\/p>\n<p>To contact a ward, phone our switchboard on (07) 36317400 and ask to be put through.<\/p>\n<h2 data-fontsize=\"28\" data-lineheight=\"33\">Parking<\/h2>\n<p>Parking is available on-site at the Brighton Health Campus.<\/p>\n"}]}],"what_to_expect":[{"expect":[{"expect_content":[{"heading":"","content":"<p>Within the first few days on the unit, the healthcare team will conduct various assessments and will work with you and your family to understand your needs and goals.
