The Complex Needs Service provides comprehensive and coordinated specialist multidisciplinary and medical assessment to identify the client\u2019s current care needs and develop an individualised care plan. Interventions focus on addressing factors that limit client\u2019s wellbeing and ability to manage their complex care needs and linking clients with sustainable longer term supports.<\/p>\n","conditions_treated":"","our_services":"<p>The Complex Needs Service has an extensive team of health professionals providing specialist care including:<\/p>\n<ul>\n<li>Dietitians<\/li>\n<li>Neuropsychologists<\/li>\n<li>Occupational therapists<\/li>\n<li>Pharmacists<\/li>\n<li>Physiotherapists<\/li>\n<li>Psychologists<\/li>\n<li>Social Workers<\/li>\n<li>Speech Pathologists<\/li>\n<li>Nurses<\/li>\n<li>Geriatricians<\/li>\n<li>General Physicians<\/li>\n<\/ul>\n","include_link_to_healthcare_professionals_section":false,"gd_other_content":false}],"accessing_this_service":[{"variation":"GP or medical practitioner referral","free_text":"<p>Referrals are made via the Community and Oral Health Central Referral Unit (Tel: 1300 658 252).
