fusion-text-1\"><p><b><i>Community and Oral Health provides accessible, culturally safe and compassionate care to patients in their home, at our oral health clinics and mobile dental vans, community and residential facilities, and bedded services.<\/i><\/b><\/p>\n<p>We are committed to delivering convenient care in the community which provides a safe alternative to hospital treatment, and helps people avoid being re-admitted to hospital.<\/p>\n<p>We support people with longer-term complex medical conditions to improve their overall health and wellbeing, dental hygiene and independence in the community.<\/p>\n<p>The community and oral health services we deliver include:<\/p>\n<ul>\n<li>Residential aged care at two nursing homes at Brighton and Kippa Ring<\/li>\n<li>Oral health care across multiple adult, adolescent and children\u2019s clinics and mobile sites<\/li>\n<li>Community-based, home visiting and bedded rehabilitation services<\/li>\n<li>Palliative care in the home<\/li>\n<li>Complex chronic disease and healthy aging care at our community facilities or in the home<\/li>\n<li>Care in the home<\/li>\n<li>Transitional bedded and community-based services for people returning home or entering a nursing home.<\/li>\n<\/ul>\n<p>Our patients and residents receive great care from our highly trained, multidisciplinary teams of medical, nursing, specialist oral post-hospital care and support including:\n\n\n\n\n\n\n\n\n\n\n<ul>\n\t\n\n\n\n<li>nursing and functional assessments<\/li>\n\n\n\n\n\t\n\n\n\n<li>occupational therapy<\/li>\n\n\n\n\n\t\n\n\n\n<li>physiotherapy<\/li>\n\n\n\n\n\t\n\n\n\n<li>nutrition support<\/li>\n\n\n\n\n\t\n\n\n\n<li>wound management.<\/li>\n\n\n\n\n<\/ul>\n\n\n\n\n<\/li>\n\n\n\n\n\t\n\n\n\n<li>About 700 people requiring intensive therapy following a serious trauma incident such as a car accident.<\/li>\n\n\n\n\n\t\n\n\n\n<li>More than 1,800 older people who had to undertake low-intensive therapy to enhance their general function and independence following discharge from a hospital.<\/li>\n\n\n\n\n\t\n\n\n\n<li>More than 1,600 patients were able to be treated safely at home through the Hospital in the Home service.<\/li>\n\n\n\n\n\t\n\n\n\n<li>Close to 550 elderly people who no longer required care in a hospital and needed support to move into a nursing home.<\/li>\n\n\n\n\n\t\n\n\n\n<li>About 500 people who received short-term care following hospital to help by September 2018.<\/li>\n<li>Increase Aboriginal and Torres Strait Islander staff representation levels from five per cent, by one per cent per year until 2020.<\/li>\n<li>Provide scholarships to
