Everyone will have access to care or treatment regardless of their vaccination status. Anyone making healthcare decisions for a patient, for example a parent or carer, will also be permitted to accompany them.<\/p>\n<p>Please check the <a href=\"https:\/\/www.qld.gov.au\/health\/conditions\/health-alerts\/coronavirus-covid-19\/current-status\/public-health-directions\/hospital-visitors\">Queensland Health hospital visitors information<\/a> for specific entry criteria before you arrive.<\/p>\n<h2>Unvaccinated visitors<\/h2>\n<p>Unvaccinated or not fully vaccinated visitors may be allowed to enter the hospital if they meet the criteria for:<\/p>\n<ul>\n<li>A parent, guardian or carer of a patient under the age of 16 years<\/li>\n<li>Supporting a patient at the end of life<\/li>\n<li>A support person for a patient during childbirth<\/li>\n<li>A carer or support person<\/li>\n<li>Supporting a patient in an emergency situation.<\/li>\n<\/ul>\n<div id=\"c19-visitor-info\">\n<div class=\"covid-toggle\" style=\"display: none;\">\n<ul>\n<li>You are unwell<\/li>\n<li>You have been diagnosed with COVID-19 or have been asked to isolate or home-quarantine<\/li>\n<li>You have been tested for COVID-19 and are awaiting test results (unless your test has been taken in accordance with a surveillance testing obligation)<\/li>\n<li>You have been informed you are a close contact of a diagnosed person or a secondary contact of a person with COVID-19<\/li>\n<li>You have a fever (37.5 degrees or more) or history of fever (e.g. nights sweats, chills) or acute respiratory infection (e.g. cough, shortness of breath, sore throat), runny nose, vomiting or nausea, diarrhoea, loss of smell or loss of taste.<\/li>\n<li>You have returned from overseas (other than a safe travel zone country) or an interstate exposure venue in the last 14 days and do not have an exemption to visit.<\/li>\n<li>You have returned from a hotspot in the last 14 days, are not fully vaccinated and do not have an exemption to visit.<\/li>\n<\/ul>\n<p><strong>If you are unsure if visitor restrictions apply to you \u2013 please call ahead and ask the ward or facility before you arrive.<\/strong><\/p>\n<\/div>\n<\/div>\n<\/div><div class=\"fusion-reading-box-container reading-box-container-1 callout callout-covid\" style=\"--awb-title-color:#05214d;--awb-margin-top:20px;--awb-margin-bottom:36px;\"><div class=\"reading-box\" style=\"background-color:#fff9e2;border-width:0px;border-color:#f6f6f6;border-style:solid;\"><div class=\"reading-box-additional\">\n<p>Unvaccinated visitors\u00a0can apply for hospital entry\u00a0by completing a\u00a0Visitor Approval Form.<\/p>\n<p>Requests will be assessed on a case-by-case by the treating facility.\u00a0Conditions apply to approved visitors.<\/p>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-text fusion-text-2\"><h2>Other Visitor Approvals<\/h2>\n<p>Visitors who meet one or more of the following criteria will need to apply for hospital entry by completing an exemption through the <a href=\"https:\/\/healthserviceportal.health.qld.gov.au\/hdsp\">Chief Health Officer<\/a>.<\/p>\n<ul>\n<li>Diagnosis of COVID-19 within the past 14 days<\/li>\n<li>Close contact of a person diagnosed with COVID-19<\/li>\n<li>Tested for COVID-19 and not yet received a result<\/li>\n<li>International arrivals within past 14 days<\/li>\n<\/ul>\n<h2>COVID-19 positive or close contact patients<\/h2>\n<p>If the patient that is being visited is a diagnosed case of COVID-19 or is a close contact and you meet one of the below criteria, please complete a <a href=\"https:\/\/forms.office.com\/r\/V5JPndbren\" target=\"_blank\" rel=\"noopener\"><strong>Visitor Approval Form<\/strong><\/a><\/p>\n<ul type=\"disc\">\n<li>A parent, guardian or carer of a patient under the age of 16 years<\/li>\n<li>Exceptional reasons or an end of life visit<\/li>\n<\/ul>\n<p><!--\n\n\n\n\n<h3>International arrivals<\/h3>\n\n\n\n\nIf you have arrived in Queensland for overseas in the last 14 days, you will need to <a href=\"https:\/\/www.qld.gov.au\/health\/conditions\/health-alerts\/coronavirus-covid-19\/current-status\/public-health-directions\/exemptions\">apply for an exemption<\/a> from Queensland Health to enter a hospital facility.--><\/p>\n<h2>Double COVID-19 vaccination required to attend our facilities for work<\/h2>\n<p>The Chief Health Officer\u2019s \u00a0<strong><a href=\"https:\/\/www.health.qld.gov.au\/system-governance\/legislation\/cho-public-health-directions-under-expanded-public-health-act-powers\/workers-in-healthcare-setting\">Workers in Healthcare Direction<\/a><\/strong> mandates any worker, who attends a Metro North facility or campus for any reason, must be double-vaccinated for COVID-19 from 15 December 2021. This includes attending our campus to undertake a service, meeting or to facilitate any kind of work, even if it is not in a clinical setting. Please familiarise yourself with the current directive and speak to your relevant Metro North contact about any questions you may have, before attending our sites.<\/p>\n<h2 class=\"\" data-fontsize=\"30\" data-lineheight=\"36px\">Virtual Visits<\/h2>\n<p>We encourage virtual visits for our patients. Virtual visits are a great way to stay connected to your loved ones and support them during their stay in our facilities The information on our\u00a0<a visits.<\/p>\n<\/div><style>\n.callout-covid { border-left: 4px solid #ffcb05!important; } \n<\/style><div class=\"fusion-clearfix\"><\/div><\/div><\/div><\/div>\n<div class=\"fusion-text fusion-text-3\"><h1>Nurse Practitioner Chronic Wound and Stoma Service<\/h1>\n<div id=\"brighton-brain-intro\">\n<p>The Nurse Practitioner Chronic Wound and Stoma Service, which is led by a highly specialised Nurse Practitioner, is open to broader referrals by General Practitioners for more complex care needs across the community.<\/p>\n<p>The Wound and Stoma Nurse Practitioner provides clinical care for the prevention, assessment and management of wounds and stomal care in a clinic setting at Chermside Community Health Centre.<\/p>\n<p>The Nurse Practitioner will implement a care plan and work with GP nurses, NGO\u2019s, patients and their families, to support the patient in the community setting.<\/p>\n<\/div>\n<\/div><div class=\"fusion-text fusion-text-4\"><div id=\"brighton-brain-access\">\n<h2>Eligible criteria for the patient:<\/h2>\n<ul>\n<li>Medicare eligible<\/li>\n<li>Over 18 years of age (under 18 must be accompanied by a guardian)<\/li>\n<li>Reside in MN geographical boundaries<\/li>\n<li>Complex or chronic wounds<\/li>\n<li>Frequent cellulitis presentations<\/li>\n<li>Skin integrity concerns such as venous stasis eczema<\/li>\n<li>Stoma complications (for example: peristomal skin concerns, hernia\u2019s, prolapse)<\/li>\n<li>General review for ostomate requiring change to product or script for products to gain access to the Stoma Appliance Scheme (via Pharmaceutical Benefits Scheme) for Ostomates<\/li>\n<li>Step down from Post-Acute Services (referral via CNC-Wound-Stoma only).<\/li>\n<\/ul>\n<p><strong>** <\/strong><em>Patients must be ambulant or be able to transfer independently<\/em><\/p>\n<p><em>** We are unable to accept bariatric patients due to equipment capacity\/ limitations<\/em><\/p>\n<h2>Ineligible criteria:<\/h2>\n<ul>\n<li>Home visits, Residential Aged Care visits<\/li>\n<li>Hospital patients eligible for post-acute care service (PACS)<\/li>\n<li>Those with Negative Pressure Wound Therapy (PACS eligible)<\/li>\n<li>Diabetic, neuropathic, neuro-ischaemic foot ulcers<\/li>\n<li>Ischemic limb, proven arterial ulcerations<\/li>\n<li>Skin cancers or fungating tumours requiring surgical intervention<\/li>\n<li>Patients accepted and waiting for specialist appointments \u2013 ie vascular or plastics (unless referred by the specialists)<\/li>\n<\/ul>\n<\/div>\n<\/div><div class=\"fusion-clearfix\"><\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-1
