Common challenges in primary care education series: Community health – a GP’s guide

Welcome to a new CPD triennium! To kick off the year and continue the theme of common challenges in general practice, we have invited the Community Health Care team to network with GPs for the evening.

There will be presentations to illustrate, in cases, how to best utilise Community Health resources for your patients. This will cover, amongst other topics:

• Nurse navigation
• Palliative care
• Post-acute care
• Rehabilitation services
• Wound care
• Diabetes
• Transitional care

There will be plenty of opportunity to ask questions in a Q & A session. Feel free to bring in cases of your own to discuss.
