Achieving great things across our service lines<\/h2>\n

\"OralAs a directorate, we are continuing to improve access to care, increase care closer to home and support patient flow across Metro North Health.\u00a0\u00a0<\/strong><\/p>\n

The Complex Chronic Disease Team (CCDT) partnership with the Australian Catholic University (ACU) continues to enhance the ability to provide a cardiac and pulmonary rehabilitation service in the northern suburbs.<\/p>\n

The cardiac and pulmonary programs have expanded to the Burpengary Regional Aquatic Leisure Centre, where clients can now receive their care face-to-face or virtually.<\/p>\n

Executive Director Glynis Schultz said in addition, despite the shortage of dental practitioners, Oral Health activity is 4.3 per cent above target this year.<\/p>\n

“The Enhanced Post-Acute Care Service has received 363 referrals from the Queensland Ambulance Service, general practitioners, emergency departments and acute inpatient services from February,” she said.<\/p>\n

Glynis said the Hospital in the Home (HITH) program had expanded and cared for patients across 328 Diagnoses Related Groups (DRGs), 19 specialities and 124 specialists.<\/p>\n

Since March 2021, 650 residents across the 85 Residential Aged Care Facilities (RACF) in the Metro North catchment area have engaged with the Specialist Palliative Care in Aged Care (SPACE) program.<\/p>\n

COH has also increased capacity of the Community Transition Care Program (CTCP).<\/p>\n<\/div><\/div><\/div><\/div>

Connecting with Country \u2013 Moora View Healing Garden Opening<\/h2>\n

Community and Oral Health hosted the Metro North Health Equity Strategy – Implementation Phase community consultation and the opening of the Moora View Healing Garden last month.<\/strong><\/p>\n

The Moora Healing Garden is a welcoming space. Its creation involved many hard-working people, led by Michelle Crawford, the Nurse Unit manager of Brighton Brain Injury Service.<\/p>\n<\/div><\/div><\/div>

Moora View Healing Garden Opening<\/p><\/div>\n

In late 2019, the idea of having a garden space accessible to patients, families and staff was first raised by a family member of a patient in Brighton Brain Injury Service (BBIS).\u00a0\u00a0<\/strong><\/p>\n

The idea was further developed and completed via patients, residents, families, staff, volunteers, the Men\u2019s Shed, Aboriginal and Torres Strait Islander staff, Elders and the community.<\/p>\n

The garden signage acknowledges the strengths of Aboriginal and Torres Strait Islander knowledges of the flora, fauna and place and the connection to Country that has spanned many millennia.<\/p>\n

Plants have an important place in Aboriginal and\/or Torres Strait Islander knowledge as food, medicine, adornment, ceremonial objects, clothing, shelter, tools artwork and spiritual beliefs.<\/p>\n

The Traditional Owners of this area call the area (now called Shorncliffe, Sandgate and Brighton) \u201cWarra\u201d, which means expanse of water and Moreton Bay \u201cMoora\u201d, meaning open sheet of water.<\/p>\n<\/div><\/div><\/div><\/div>

Our Community Palliative Care Service has partnered with Ambulance Wish Queensland to deliver our first ever patient wish.<\/strong><\/p>\n

A big thank you to everyone who helped fulfill Ross\u2019s wish, which was to have his mum Eileen transported to see him at his home.<\/p>\n
