This care is aligned with the patient\u2019s expressed goals of care and healthcare needs, considers the effect of the patient\u2019s health issues on their life and wellbeing, and is clinically appropriate.<\/p>\n<p><img decoding=\"async\" class=\"alignright wp-image-15035 lazyload\" data-src=\"https:\/\/metronorth.health.qld.gov.au\/extranet\/wp-content\/uploads\/sites\/10\/2023\/06\/standard-5-icon.jpg\" alt=\"Standard 5 icon\" width=\"200\" height=\"178\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 200px; --smush-placeholder-aspect-ratio: 200\/178;\" \/>To ensure that risks of harm for patients during health care are prevented and managed. Clinicians identify patients at risk of specific harm during health care by applying the screening and assessment processes required in this standard.<\/p>\n<p><strong>COH priorities<\/strong><\/p>\n<ul>\n<li>Active involvement of patients in their own care by discussing and documenting goals of care and including patients during bedside handover.<\/li>\n<li>Completion of risk assessments and management plans\/actions.<\/li>\n<li>Preventing falls and harm from falls.<\/li>\n<li>Preventing pressure injuries.<\/li>\n<\/ul>\n<p><strong>What can you do?<\/strong><\/p>\n<ul>\n<li>Ensure risk assessments are complete and goals of care are documented.<\/li>\n<li>Be person centred in your practices.<\/li>\n<li>Know the patient\u2019s support people and make them feel welcome.<\/li>\n<li>Share information openly with other clinicians.<\/li>\n<li>Understand the procedures that support comprehensive care \u2013 Delirium, Restraint, Bed Rails, Pressure Injury, Falls and Nutrition.<\/li>\n<li>Contribute ideas to implement strategies for improvement.<\/li>\n<\/ul>\n<h3>Standard 8: Recognising and responding to acute deterioration (Lead &#8211; Stephanie Hadikusumo)<\/h3>\n<p><strong>Standard intention<\/strong><\/p>\n<p><img decoding=\"async\" class=\"alignright wp-image-15038 lazyload\" data-src=\"https:\/\/metronorth.health.qld.gov.au\/extranet\/wp-content\/uploads\/sites\/10\/2023\/06\/standard-8-icon.jpg\" alt=\"Standard-8 icon\" width=\"200\" height=\"181\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 200px; --smush-placeholder-aspect-ratio: 200\/181;\" \/>To ensure that a person&#8217;s acute deterioration is recognised promptly and appropriate action is taken.
