In 2023, we are taking some very important and exciting steps to prepare Home Hospital services for future demand and sustainability.<\/h4>\n

We have developed the Home Hospital Future QHEPSs site<\/a>\u00a0to provide important information and updates for staff as we actively improve the stream and deliver on the findings of a broad review of Home Hospital services which occurred in early 2023.<\/p>\n

This site outlines details of those findings, our phased approach, upcoming steps and consultation activity as we undertake a body of work over the next 12 months to carefully consider all the recommendations of the review for Home Hospital services.<\/p>\n

Home Hospital Future – All you need to know!<\/a><\/p>\n

COH Volunteer Program Compliment – Glenn Ellis<\/h3>\n\n\n\n
While Glenn Ellis has been a Community and Oral Health (COH) volunteer for only a short time, the appreciation being expressed by our patients and staff has been coming thick and fast.<\/p>\n

\u201cI have been a patient in the Rehabilitation Unit for around 11 weeks, and I was introduced to volunteer Glenn about six weeks ago.<\/em><\/p>\n

\u201cGlenn introduced himself and explained the purpose of his role as a volunteer and made me feel immediately comfortable.<\/em><\/p>\n

\u201cFrom the time I met Glenn, I had something to look forward to.\u00a0I looked forward to having Glenn visit me on Monday mornings.<\/em><\/p>\n

\u201cI also had the opportunity to have an outing from the ward to the Sandgate and Districts Men\u2019s Shed, which I thoroughly enjoyed.\u00a0This opportunity was a \u2018huge plus\u2019 as it provided me with opportunity to better myself.<\/em><\/p>\n

\u201cI have a background as a builder, so I felt I could also add value and could contribute to what the Men\u2019s Shed offers.<\/em><\/p>\n

\u201cVolunteers like Glenn are essential and provide good value to the care provided. Having someone to engage with face-to-face is extremely beneficial for people\u2019s health and wellbeing.<\/em><\/p>\n

\u201cGlenn is a great representative to your service, and you are extremely lucky to have such a professional volunteer onboard. Glenn\u2019s weekly visits were of great value and benefit to me.\u201d<\/em><\/p>\n

Join the COH Reconciliation Action Group<\/h3>\n

Community and Oral Health are inviting new members interested in joining our COH Reconciliation Action Plan (RAP) Working Group.<\/p>\n

We are seeking staff who are committed and interested in reconciliation and developing stronger relationships between Indigenous and non-Indigenous Australians and how such relationships can be a positive force for changing the culture, work practices and core business of COH.<\/p>\n

The COH Reconciliation Action Plan Working Group drives the development of\u00a0a Stretch RAP within the context of COH core business and Reconciliation Australia\u2019s RAP framework.<\/p>\n

It is intended that the working group will meet bi-monthly, commencing August 31\u00a0and members are required to be present for at least 90 percent of the meetings\/workshops.<\/p>\n

Rapid access having remarkable success<\/h3>\n

The Rapid Access to Community Care (RACC) service<\/a> is having remarkable success supporting chronically ill elderly patients stay at home and out of hospital.<\/p>\n

The service, which assesses patients in the home so referrals to community or home support services can be fast tracked, has assisted around 400 adult patients who have experienced significant deterioration in their chronic disease or illness since the beginning of the year.<\/p>\n

Assessments are being conducted within 24 hours of a referral, and the RACC team will provide a comprehensive assessment of health, functional capabilities and the ongoing support needs of the patient.<\/p>\n

So far, close to 150 GP practices in Brisbane North have engaged the service.<\/p>\n
