--smush-placeholder-aspect-ratio: 400\/267;\" \/><p id=\"caption-attachment-3502\" class=\"wp-caption-text\">The Community Transition Care Program\u2019s GEM@Home trial was highly commended in the integrated care category of the Community and Oral Health Staff Excellence Awards.<\/p><\/div>\n<p><span data-contrast=\"auto\">\u201cThe Metro North Geriatric Evaluation and Management at Home (GEM@Home) was a short\u00a0three month\u00a0trial which provided a community-based, geriatrician-led service, which aimed to minimise hospital stay and support the ongoing care needs of the older person in their own home,\u201d\u00a0she\u00a0said.<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cThe\u00a0GEM@Home\u00a0trial supported early transfer of older patients from hospital and built clear pathways for the ongoing management of age-related conditions and reduced hospital readmissions.<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cWe saw a four to five-day reduction in length of stay in hospital for the trial cohort, plus a decrease in unplanned readmissions.\u201d<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">As part of GEM@HOME, 26 patients from The Prince Charles Hospital who had an average age of 85 years, safely left hospital early and received care at home.<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cPersons admitted to the trial mainly included those that had a fall, or needed mobility support, personal care, nutrition, medication management, pressure injury care or other medical requirements,\u201d\u00a0Leanne\u00a0said.<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cWe also saw a significant cost in the reduction per episode of care with an estimated 2:1 cost ratio difference between hospital care compared to safe care in the home.\u201d\u202f\u00a0<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The trial saw five additional Hospital in the Home places dedicated to\u00a0older\u00a0patients, following their stay in The Prince Charles Hospital.<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Leanne\u00a0said\u00a0patients received home access to a multidisciplinary team, including a dedicated geriatrician, nurses, physiotherapists, occupational therapists, dieticians, social workers,\u00a0pharmacists\u00a0and speech therapists.<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cThe TPCH service continues with the view of developing a broader service to support elderly people at home across Metro North,\u201d she said.<\/span><span data-ccp-props=\"{\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">GEM@HOME was led by Metro North Health staff from\u00a0the\u00a0Community Transition Care Program,\u00a0and Innovation and Research Centre, as well as The Prince Charles Hospital.\u00a0<\/span><\/p>\n<\/div><\/div><\/div><\/div><\/div>\n","protected":false},"excerpt":{"rendered":"<p>A new, innovative trial has safely supported the acute\u202fand long-term care needs of older people in Metro North who have complex and chronic health conditions associated with ageing. <\/p>\n<p>Community and Oral Health Transition Care Team Leader Leanne Carroll said across Metro North we have seen an increased demand on community services to provide healthcare at home that supports healthy ageing and the care needs of the elderly. <\/p>\n","protected":false},"author":51,"featured_media":3502,"comment_status":"open","ping_status":"open","sticky":false,"template":"","format":"standard","meta":{"_acf_changed":false,"_searchwp_excluded":"","footnotes":""},"categories":[174],"tags":[],"class_list":["post-3545","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-coh-express"],"acf":[],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v26.2 - https:\/\/yoast.com\/wordpress\/plugins\/seo\/ -->\n<title>HIGHLY COMMENDED \u2013 Community trial enhances care for the older person - Staff Extranet<\/title>\n<meta name=\"robots\" content=\"noindex, follow\" \/>\n<meta property=\"og:locale\" content=\"en_US\" \/>\n<meta property=\"og:type\" content=\"article\" \/>\n<meta property=\"og:title\" content=\"HIGHLY COMMENDED \u2013 Community trial enhances care for the older person - Staff Extranet\" \/>\n<meta property=\"og:description\" content=\"A new, innovative trial has safely supported the acute\u202fand long-term care needs of older people in Metro North who have complex and chronic health conditions associated with ageing.
