The whole Zillmere Residential Transition Care Program team at Zillmere are making such a difference to our patients \u2013 and are demonstrating the value of teamwork.<\/p><\/div>\n

It is always a treat when a service line receives some creative and heartfelt artwork or crafts from a patient or family that are so grateful for the care they receive.<\/strong><\/p>\n

And, this occurs regularly for the Zillmere Residential Transition Care Program team, for clear reason \u2013 they are a great team.<\/p>\n

Thank you, Ella, for your wonderful words and hand-crafted masterpiece that captured pretty much the whole multidisciplinary team at Zillmere Residential Transition Care Program.<\/p>\n

    The Zillmere Residential Transition Care Program is for the older person who requires more time and support to complete their restorative care after a hospital stay.<\/p>\n

    The program is delivered in a non-hospital residential setting at Zillmere and supports people to improve their functional capacity and finalise and access their longer-term care arrangements.<\/p>\n

    Fast facts \u2013 Zillmere Residential Transition Care Program<\/strong><\/p>\n

    • More than 80 per cent of our patients are aged 70 years or over<\/li>\n
    • 63 per cent of our patients are women<\/li>\n
    • Close to 40 per cent of patients return straight home after their admission<\/li>\n
