Your discharge plan may include extra support services at a different facility at home or at follow-up appointments.<\/p>\n<h2>Leaving hospital (discharge)<\/h2>\n<p>You may be discharged at 10.00am. Some patients may stay until later in the day depending on transport arrangements.\u00a0If you have any questions or concerns, please talk to your care team. Before you leave the hospital, please check that you have:<\/p>\n<ul>\n<li>arranged transport home<\/li>\n<li>collected all your personal belongings, x-rays and medications<\/li>\n<li>your discharge medications and any scripts required<\/li>\n<li>details for your follow-up appointments (if necessary)<\/li>\n<li>a medical certificate for your employer or sickness benefits if required<\/li>\n<li>a medical discharge summary for your GP<\/li>\n<li>any referral forms you need (if services have been arranged for you).<\/li>\n<\/ul>\n<h2>Transport and transfers home<\/h2>\n<p>You will need to arrange private transport home. Ambulance transport will be organised if your care team determines it is necessary.\u00a0Hospital walking aids and other equipment are not allowed outside the hospital grounds. Your care team or your family will need to make arrangements to hire appliances or aids from a private supplier if you need them at home.<\/p>\n<p>A public bus service between Kilcoy and Caboolture leaves daily from the bus depot in William Street, Kilcoy. Phone (07)&nbsp;5497&nbsp;1478 for a timetable.<\/p>\n<p>Kilcoy taxis can be contacted on 0427&nbsp;073&nbsp;571<\/p>\n<h2>Medication when you leave<\/h2>\n<p>Make sure you know which medications to take once you are discharged. The medications you bring to the hospital with you may not be suitable to take after your hospital stay.\u00a0During your stay in hospital you will receive an invoice for medications that may be ordered for you for when you leave hospital. You will need to pay for these medications upon discharge.<\/p>\n<h2>Concerns after you leave hospital<\/h2>\n<p>Contact your GP or 13 HEALTH (13 43 25 84) if you have any problems after you leave the hospital. If you need urgent care call Triple Zero (000) or visit your local Emergency Department.<\/p>\n<h2>Medical records<\/h2>\n<p>Your medical record is confidential and is accessed only by healthcare professionals directly involved with your treatment. The medical record is the property of the hospital, and the information contained in the record will normally only be provided with your consent or where required by law. Find out how to <a href=\"https:\/\/metronorth.health.qld.gov.au\/about-us\/information-access-privacy\/accessing-health-records\">access your medical record<\/a>.<\/p>\n<h2>Modifying your home<\/h2>\n<p>After your hospital visit, you may need to make some changes in your home so you can carry out daily activities safely. Your Allied Health therapist may talk with you and suggest changes, equipment or additional support.<\/p>\n<h2>Follow up appointments (outpatient appointments)<\/h2>\n<p>Your doctor will notify you if you are required to have a follow up\u00a0<a href=\"https:\/\/metronorth.health.qld.gov.au\/kilcoy\/patients-and-visitors\/your-appointment\">outpatient appointment<\/a>. This can be 6-8 weeks after your procedure.<\/p>\n<h2>Ongoing care<\/h2>\n<p>Before you leave hospital, we will create a discharge care plan with you.
