culturally safe and respectful care to Aboriginal and Torres Strait Islander respiratory patients aged 16 years and over, with the aim to improve respiratory health.<\/p>\n<p>Aboriginal and Torres Strait Islander patients who attend the UROC clinics will have access to specialty lung investigations and be able to see a specialist respiratory physician.<\/p>\n<p>The clinics will assist in early Clinic (HAC) provides specialist geriatrician services on Brisbane Northside for Aboriginal and Torres Strait Islander people over 50 years of age.<\/p>\n<p>The clinics are delivered at Moreton Aboriginal and Torres Strait Islander Community Health Service (MATSICHS) in collaboration with Metro North Health.<\/p>\n<p>HAC provides specialised care in a culturally safe environment for Aboriginal and Torres Strait Islander older persons with chronic or complex conditions relating to ageing, frailty, disability, and cognitive problems.<\/p>\n<p>This includes:<\/p>\n<ul>\n<li>Cognitive and functional assessment<\/li>\n<li>Restorative care assessment<\/li>\n<li>Comprehensive geriatric assessment<\/li>\n<li>Dementia diagnosis and management<\/li>\n<li>Falls risk assessment and management<\/li>\n<li>Polypharmacy<\/li>\n<\/ul>\n<h3>Who can access the service?<\/h3>\n<ul>\n<li>Patients 50 years of age and older who identify as Aboriginal and\/or Torres Strait Islander.<\/li>\n<li>Immediate family members (even if the immediate family member does not identify as an Aboriginal and\/or Torres Strait Islander person) defined as spouse, partner, sibling or child of someone of Aboriginal and Torres Strait Islander origin.<\/li>\n<\/ul>\n<h3>How to access this service<\/h3>\n<ul>\n<li>Access to HAC can be by a referral from with:<\/p>\n<ul>\n<li>Understanding your pain and how to best manage it.<\/li>\n<li>Accessing services to ease your pain.<\/li>\n<li>Access to specialist treatment.<\/li>\n<\/ul>\n<p>The service is a partnership between MATSICHS and the Tess Cramond Pain and Research Centre. The team includes:<\/p>\n<ul>\n<li>Pain Medicine Consultants<\/li>\n<li>Nurse Care Coordinators\/Nurse Navigators<\/li>\n<li>Psychologists<\/li>\n<li>Occupational Therapists<\/li>\n<li>Physiotherapists<\/li>\n<li>Social Workers<\/li>\n<li>Pharmacists<\/li>\n<\/ul>\n<h3>What is persistent pain?<\/h3>\n<p>Pain may be felt after illness, injury or surgery. Sometimes there isn\u2019t an obvious reason. We call it persistent when it has been there for more than we would expect for our body to heal (usually more than three months).<\/p>\n<h3>What happens at the service?<\/h3>\n<p>Your care pathway will depend on what is going on with you.<\/p>\n<p>You will be contacted by the team, who will yarn about how we can support you, and what to expect.
