Working together to connect care program
We work across hospital departments, primary care and community organisations to effectively respond to individuals who commonly present to the Emergency Department at the Royal Brisbane and Women’s Hospital.
Through partnering with community organisations such as Footprints, Micah Projects and the Institute for Urban Indigenous Health, we are able to connect reoccurring emergency patients to the care they need more efficiently.
If you would like to speak to a clinician about the program to:
- discuss suitability of the program for your patient
- discuss a patient who is already on the program
- obtain information in relation to the program
- confirm a patient has withdrawn consent
- general enquiry

How to join the program
Call us to discuss if this program is suitable for your patient and/or to organise consent for the program. Please note, patients can commence the program without consent however they will not be referred to a community organisation until consent is obtained.
