I'd like to specifically thank\u00a0project officer Megan Husband who got everything up and running and Belinda Faulkner who will continue to oversee the program.\u00a0<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/p>\n<p><b><span data-contrast=\"auto\">Comprehensive Care Standard (Standard 5)<\/span><\/b><span data-ccp-props=\"\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The National Safety and Quality Health Service Standards (NSQHS) Standard 5 Comprehensive Care has been reviewed this month across\u00a0Metro North\u00a0by the Clinical Governance, Safety, Quality and Risk team.<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The intent of the Comprehensive Care standard (Standard 5) is to ensure that the patient receives care that is planned and coordinated around their physical, mental, and cognitive health needs. The standard aims to ensure that a patient's goals and risks of harm are identified so that comprehensive care plans can be developed and delivered to meet their needs. The standard also identifies that patients should continue to receive comprehensive and compassionate care at the end of their life (Australian Commission on Safety and Quality in Health Care, 2017).<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The quality and safety review undertaken outlines the relationship between the key components of the Comprehensive Care standard and how Metro North implements systems and processes of governance, and quality improvement methods to provide continuous and collaborative care, while working in partnership with patients, carers and families to adequately identify, assess, and manage patients' clinical risks, and establish their preferences of care.<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The\u00a0Comprehensive Care\u00a0standard is informed by four overarching criteria. These criteria provide the framework for\u00a0our\u00a0Metro North\u00a0reporting\u00a0and include:<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/p>\n<ul>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"1\" aria-setsize=\"-1\" data-aria-posinset=\"1\" data-aria-level=\"1\"><span data-contrast=\"auto\">Clinical governance and quality improvement to support comprehensive care,<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/li>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"1\" aria-setsize=\"-1\" data-aria-posinset=\"2\" data-aria-level=\"1\"><span data-contrast=\"auto\">Developing the comprehensive care plan,<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/li>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"1\" aria-setsize=\"-1\" data-aria-posinset=\"3\" data-aria-level=\"1\"><span data-contrast=\"auto\">Delivering comprehensive care,<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/li>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"1\" aria-setsize=\"-1\" data-aria-posinset=\"4\" data-aria-level=\"1\"><span data-contrast=\"auto\">Minimising patient harm.<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/li>\n<\/ul>\n<p><span data-contrast=\"auto\">Some key patient quality and safety improvements achieved under standard 5\u00a0in\u00a0Metro North\u00a0include a continued downward trend for the number of coded stage 3 hospital\u00a0acquired pressure injuries,\u00a0with a 33% decrease in the period April 2020 to March 2021 when compared to\u00a02019\/2020.\u00a0Pressure injury prevention and management strategies\u00a0include\u00a0skin inspection within\u00a0eight\u00a0hours\u00a0of admission, pressure injury risk assessment within\u00a0eight\u00a0hours, and\u00a0a\u00a0pressure injury prevention plan documented, all remain consistently above 92% for the past 2 years.<\/span><span data-ccp-props=\"\">\u00a0<\/span><span data-ccp-props=\"\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Rates of Hospital Acquired Malnutrition incidences have decreased by 49% compared to the previous year.
