Social Workers assist people by helping to address psychosocial issues that may be impacting on their health.<\/p>\n","conditions_treated":"","our_services":"<p>We provide a range of specialised and generalist services to support you and your family while at STARS.<\/p>\n<p>Our services<em>\u00a0<\/em>include:<\/p>\n<ul>\n<li>Advisory role with difficult to solve practical problems.<\/li>\n<li>Working through difficult decisions that may be impacting on health.<\/li>\n<li>Assisting with psychosocial issues regarding recovery and return to normal activity post treatment.<\/li>\n<li>Crisis intervention.<\/li>\n<li>Developing and coordinating discharge plans from hospital appropriate to an individual\u2019s needs.<\/li>\n<li>Grief counselling.<\/li>\n<li>Identifying appropriate resources and linkages within the community.<\/li>\n<li>Providing counselling to individuals and families.<\/li>\n<li>Providing information about the psychosocial impact of illness.<\/li>\n<li>Providing information and education about the health care system.<\/li>\n<li>Advance Care planning education.<\/li>\n<li>Assistance in transitioning to
