After discharge, patients can be seen at one of our Specialist Diabetes Outpatient Clinics closest to their home, this includes:<\/p>\n
No external referrals are accepted for the Inpatient Diabetes Service. You can only access this service when you are staying in hospital or if you have been referred by a hospital clinician.<\/p>\n
To access the Specialist Diabetes Outpatient Clinic, your GP or medical practitioner will need to send a referral letter to Community, Indigenous and Subacute Services (CISS).<\/p>\n
If there is a waiting list<\/a>, you will receive a confirmation letter and be advised on what to do next.
