If circumstances occur where we need you to take your own medications, we will discuss this with you first.<\/p>\n"}]},{"other_content":[{"heading":"Medication during your stay","content":"<p>Your pharmacist will ask about all medications you are currently taking and if you have any allergies.\u00a0Our pharmacists will:<\/p>\n<ul>\n<li>establish an accurate medication history on admission<\/li>\n<li>work with the healthcare team to select the most appropriate medication for your care<\/li>\n<li>offer medication advice to doctors, nurses and other healthcare professionals within the hospital<\/li>\n<li>dispense medicines for you<\/li>\n<li>explain the purpose, dose, how to use\/take your medication and possible side effects<\/li>\n<li>develop a plan for the use of your medication and when to contact your doctor.<\/li>\n<\/ul>\n<p>If you have questions about the medications you have been prescribed during your stay, please ask to speak to your pharmacist.<\/p>\n"}]},{"other_content":[{"heading":"Medication management","content":"<p>We focus on promoting the safe and effective use of medication through:<\/p>\n<ul>\n<li>medication histories on admission to hospital with a complete and accurate record of current and recently ceased medications<\/li>\n<li>inpatient medication chart review<\/li>\n<li>involvement in consultant ward rounds and outpatient clinics<\/li>\n<li>medication counselling to patients both as inpatients and on discharge<\/li>\n<li>delivery of drug information to medical staff, nursing staff and other health professionals<\/li>\n<li>therapeutic drug monitoring service<\/li>\n<li>training and ongoing education of staff and undergraduate\/postgraduate students.<\/li>\n<\/ul>\n"}]},{"other_content":[{"heading":"Do I have to pay for medications?","content":"<p>You will not be charged for medications that you receive while staying in hospital.<\/p>\n<p>The doctor may write you a prescription before you leave hospital.
