Patients needing more complex or extended kidney care may be transferred to other sites to access programs available in the MNHHS Kidney Health Service.<\/p>\n<p>We offer a comprehensive service including:<\/p>\n<ul>\n<li>general kidney outpatient community clinics<\/li>\n<li>outpatient consultations in the subspecialties of chronic kidney disease, diabetic kidney disease, dialysis, kidney supportive care, renal genetics, transplantation and vasculitis<\/li>\n<li>telemedicine delivery of care<\/li>\n<li>specialised inpatient care and consultations<\/li>\n<li>preparation of patients for kidney transplantation and post-transplant care<\/li>\n<li>training for independent haemodialysis and peritoneal dialysis<\/li>\n<li>independent dialysis, including home based dialysis, supported by an ambulatory care team<\/li>\n<li>dependent chronic centre based haemodialysis<\/li>\n<li>transition to end of kidney
