Information challenges in aged care: supporting data standardisation and exchange

Older people use health and aged care extensively, often linked to multiple service providers simultaneously.

Continuity of care is ideally supported by shared information around medical, functional and psychosocial needs.  In this presentation, a series of strategies are proposed to support information sharing among providers, addressing data standardisation, information exchange and decision support.

Professor Len Gray

MBBS, Master of Medicine, PhD, Director of the Centre for Health Services, Faculty of Medicine, The University of Queensland

Len Gray is Director of UQ Centre for Health Services Research and Professor in Geriatric Medicine. He is trained in health management and as a specialist in geriatric medicine. His career has encompassed clinical practice, senior health administration roles, and the past 15 years in an academic role at UQ. Research interests include health systems organisation, assessment and quality systems, aged care in general and telehealth. He leads international development of hospital systems within the interRAI research collaborative, which creates and distributes assessment and decision support systems related to functional and psychosocial aspects of care.
