Discharge Support Service (DSS)
The Discharge Support Service (DSS) provides a contact point for patients who experience complications or concerns within 30 days of discharge from Caboolture Hospital. Our goal is to improve patient’s experience with their discharge journey.
The service is staffed by a Clinical Nurse Consultant and supported by senior Allied Health professionals. The DSS clinicians provide support via telehealth, either on the phone or videoconference. Some patients may be asked to come to hospital to be seen face to face in a clinic space.
Our services
The Discharge Support Service looks after adult patients that have been admitted to hospital for surgery or illness.
It provides rapid access to unplanned care for patients after discharge home.

How to access this service
This service is available to all patients aged 16 years and over, following discharge from a general medical or surgical admission at Caboolture Hospital.
Please note: The patient must be present during the call, as we are unable to provide support solely through carers or family members.
Support is via telehealth, either on the phone or videoconference.
