    UROC

    The Urban Indigenous Respiratory Outreach Clinic (UROC) Program, in partnership with the Institute for Urban Indigenous Health, delivers culturally safe and respectful care to Aboriginal and Torres Strait Islander respiratory patients aged 16 years and over, with the aim to improve respiratory health.

    Aboriginal and Torres Strait Islander patients who attend the UROC clinics will have access to specialty lung investigations and be able to see a specialist respiratory physician.

    The clinics will assist in early diagnosis and management of lung conditions.

    Managed respiratory conditions:

    • Asthma
    • Bronchiectasis
    • Interstitial lung disease
    • COPD
    • Other general respiratory conditions
    • Shortness of breath for investigation
    • Chronic cough
    • Pulmonary nodules

    HOPE

    The Heart Outreach Program for Health Equity (HOPE) delivers culturally safe and respectful care to Aboriginal and Torres Strait Islander cardiology patients aged 16 years and over, with aim to improve heart health.

    Aboriginal and Torres Strait Islander patients who attend the HOPE clinics will have access to speciality cardiac investigations (including ECG, Holter and Echo services), and see a specialist cardiology physician.

    The clinics will assist in early diagnosis and management of cardiovascular conditions.

    The HOPE Program is available for people who identify as Aboriginal and Torres Strait Islander, requiring a specialist cardiology service.

    Healthy Ageing Clinic

    The Aboriginal and Torres Strait Islander Healthy Ageing Clinic (HAC) provides specialist geriatrician services on Brisbane Northside for Aboriginal and Torres Strait Islander people over 50 years of age.

    The clinics are delivered at Moreton Aboriginal and Torres Strait Islander Community Health Service (MATSICHS) in collaboration with Metro North Health.

    HAC provides specialised care in a culturally safe environment for Aboriginal and Torres Strait Islander older persons with chronic or complex conditions relating to ageing, frailty, disability, and cognitive problems.

    This includes:

    • Cognitive and functional assessment
    • Restorative care assessment
    • Comprehensive geriatric assessment
    • Dementia diagnosis and management
    • Falls risk assessment and management
    • Polypharmacy

    Who can access the service?

    • Patients 50 years of age and older who identify as Aboriginal and/or Torres Strait Islander.
    • Immediate family members (even if the immediate family member does not identify as an Aboriginal and/or Torres Strait Islander person) defined as spouse, partner, sibling or child of someone of Aboriginal and Torres Strait Islander origin.

    How to access this service

    • Access to HAC can be by a referral from your GP through to a Metro North Hospital and Health Geriatric Service.
    • HAC clinics are available every Tuesday from 9am – 4.30pm.

    Persistent Pain Clinic

    The Persistent Pain Service provides Aboriginal and Torres Strait Islander people aged 16 and over, and their immediate families, access to a culturally safe service to manage ongoing pain.

    The Persistent Pain Service can support you with:

    • Understanding your pain and how to best manage it.
    • Accessing services to ease your pain.
    • Access to specialist treatment.

    The service is a partnership between MATSICHS and the Tess Cramond Pain and Research Centre. The team includes:

    • Pain Medicine Consultants
    • Nurse Care Coordinators/Nurse Navigators
    • Psychologists
    • Occupational Therapists
    • Physiotherapists
    • Social Workers
    • Pharmacists

    What is persistent pain?

    Pain may be felt after illness, injury or surgery. Sometimes there isn’t an obvious reason. We call it persistent when it has been there for more than we would expect for our body to heal (usually more than three months).

    What happens at the service?

    Your care pathway will depend on what is going on with you.

    You will be contacted by the team, who will yarn about how we can support you, and what to expect. This might include yarning with the social worker, pharmacist or nurse navigator who will work with you to understand what’s been going, and how we might be able to support you best.

    At your first appointment in the clinic, you will meet with a pain medicine consultant and might meet some other members of your care team who will listen and yarn with you about your pain journey so far.

    The service also offers a group program to focus on how to understand your pain, as well as ways to deal with your pain and take steps towards recovery.

    Women’s Business Shared Pathway

    The Women’s Business Shared Pathway program delivers culturally safe health care to Aboriginal and Torres Strait Islander women for a range of gynaecology and pelvic health conditions.

    Common conditions the program can assist with:

    • Period worries, including Polyendocrine Metabolic Ovarian Syndrome (PMOS)
    • Menopause
    • Pelvic pain including painful periods, painful sex, endometriosis
    • Bowel or bladder leakage and pelvic organ prolapse
    • Contraception
    • Other vaginal or vulval concerns
    • Dietary support for any of the above

    The team includes:

    • Aboriginal and Torres Strait Islander Health Worker/Nurse
    • Gynaecologist
    • Physiotherapist
    • Dietician
