Central Foot Hub
Your foot health, closer to home.
The Central Foot Hub is a virtual specialist multi-disciplinary service for people with foot wounds who live in the Central Queensland, Wide Bay and Central West regions.
We work alongside you and your foot health providers to help overcome barriers to healing your foot wound and optimising your health.
Our services
This includes but isn’t limited to in relation to foot wound:
- Diabetes support and management
- Support and coping strategies
- Impact on ability to work and financial stressors
- Carers Strain, connecting with available supports
- Optimising nutrition for wound healing or weight
- Stress related to having a chronic condition or adjustment to disease
- Footwear and offloading devices to take pressure away from the wound location
- Complex or reoccurring infection review.
Refer a patient
Patients who meet the following criteria can be referred to our service:
- Current foot disease or active wound (ulceration, infection, ischaemia or Charcot neuroarthropathy).
- Seeking multidisciplinary foot care team specialist input.
- Patient has consented to referral.
Email the Central Foot Hub team at foothub-central@health.qld.gov.au for a copy of the referral form.

*Vascular surgery only available for MDT case conference.

Refer a patient
GPs and community organisations can refer patients to Central Foot Hub using the Community Foot Hub referral Form and emailing the team.
