Dental prosthetic provider list

If you have received an Outsourcing Dental Voucher, please contact a provider from the list as soon as possible and advise them you have the voucher. The Dental Clinic will offer you their next available appointment. As a courtesy to the private dentist, please ensure that you keep your appointment or notify the dentist if you are unable to attend as these appointments are very limited and high in demand. If the private dentist is unable to see you in the time frame allocated on the voucher, please refer to another provider on the list.

You will need to provide the dentist with the Outsourcing Voucher, Pension or Health Care Card and one form of identification.

Please note: Funding for outsourcing is subject to availability and may not always be available. Emergency vouchers are valid for 15 days from date of issue. General or Prosthetist Vouchers are valid for 30 days from date of issue.

Once treatment has been completed and you’re satisfied with the treatment provided, your signature will be required on the voucher.
