Hospital in the Home (HITH) Service

Our Hospital in the Home service provides the same quality of care as you would receive in hospital, but in the comfort of your home or another selected location.

We work with you to develop an appropriate treatment plan to meet your care needs to help you during your recovery.

Our services

Our team will contact you on a daily basis to provide treatment and monitor your progress.

We will:

• schedule medical reviews and facilitate clinic appointments
• provide advice on what to do in an emergency
• provide telephone support after hours
• develop a discharge plan
• facilitate referrals to appropriate services as needed.

Once your condition has improved and you no longer require hospital-level treatment, your care will be transferred back to your GP.

How to access this service

No external referrals are accepted for this service. You can only access this service if you have been a patient at the hospital and have been referred by a hospital clinician.
