Cardiac Rehabilitation

The cardiac rehabilitation service provides an integrated education and exercise program for clients following a recent (within six months) cardiac event.

Cardiac rehabilitation is an exercise and education program that teaches people with heart conditions the skills they need to build healthy heart habits and to help with recovery.

Exercise can help you stay out of hospital and improve your quality of life by increasing your fitness, giving you more energy, improving your sleep, and helping you to feel happier and more relaxed.

Our services

Programs are available in the Metro North Health for patients with mild heart conditions through to advanced heart failure. Each patient is individually assessed and given a program to address their needs.

The program is conducted over 6 weeks and includes the following:

  • Exercise program – once or twice weekly in a gym setting or via telehealth in the your own home
    You will be assessed for suitability for home based virtual care delivery and only offered this option if clinically appropriate
  • Education program – varying topics related to health, diet and fitness
  • Access to multidisciplinary allied health team for identified needs/goals

The service is available at various locations throughout the Metro North catchment

How to access this service

Referrals are made via the Community and Oral Health Central Referral Unit (Tel: 1300 658 252). Referrals are accepted from hospitals, General Practitioners and other Queensland Health community services, please:

  • call CRU on 1300 658 252 if you are an external referrer

For all referrals, please attach recent investigations and clinical assessments

What to expect

The cardiac rehabilitation program is conducted over six weeks, it includes the following.

  • Group exercise program- twice weekly gym sessions for six weeks.
  • Education program – one session per week on varying topics related to health, diet and fitness delivered online or centre based

Service available at North Lakes Health Precinct, Burpengary aquatic centre & Chermside Community Health Centre

Suitable clients will be assessed for home based virtual care delivery.
