Community Based Rehabilitation Team (CBRT)
Community Based Rehabilitation Team (CBRT) aims to improve people’s well-being, independence and quality of life. People who will benefit from this service are those who have had a recent injury, illness or surgery (within the past six months) which has resulted in rapid functional decline and has impacted on their ability to complete everyday activities.
People with a recent diagnosis of:
- stroke
 - acquired brain injury
 - early stage or acute exacerbation of neurodegenerative conditions with rehabilitation capacity
 - an orthopaedic condition
 - amputation requiring functional rehabilitation, that is, improving functional capacity (not maintenance) of function.

CBRT Service delivery incorporates:
- A client centred approach utilising an individual case management framework
 - Time limited and goal orientated therapy
 - Facilitation of client and carer involvement in the rehabilitation process and to establish clear functional goals
 - Emphasis on effective and ongoing communication between the team and the client, carer and relevant stakeholders (including the client’s General Practitioner)
 
The Community Based Rehabilitation Team provides multiple services including:
- Physiotherapy
 - Occupational Therapy
 - Speech Pathology
 - Social Work
 - Leisure Therapy
 - Clinical Assistant
 - Rehabilitation Specialist

Referrals are accepted from hospitals, General Practitioners and other Queensland Health community services, please:
- use GP referral system for GPs, or

Inclusion criteria
- Ability to participate in self directed therapy program (or use carer support)
 - Recent health event (past 6 months) and / or hospitalisation with functional changes
 - Have functional rehabilitation goals with the capacity to improve with a time limited block of therapy
 - Prosthetic rehabilitation
 
Considerations
- Clients requiring single discipline intervention only, that may be better suited to an outpatient clinic
 - Ongoing active medical treatment plan that may impact on capacity to participate in therapy, eg. Cancer care patients attending chemo/radiation 3 x weekly, dialysis 3x weekly
 
Exclusion criteria
Not appropriate for CBRT
- No functional rehabilitation goals identified
 - Limited capacity to participate in therapy (eg. Acutely medically unstable, no fixed abode)
 - Non or partial weight bearing awaiting upgrade to full weight bearing status, and/or requires a hoist for transfers
 - Functional symptoms without a confirmed diagnosis (eg. FND)
 - Deconditioning in the absence of an acute medical event or hospitalisation
 
Essential referral information
- Patient medical history and current medications
 - Details of the reason for referral, including any recent hospitalisations
 - Goals for rehabilitation
 - If the client is a participant in NDIS, or receiving a home care package
 - Any further investigations the client is referred for
