The Mobile Intensive Rehabilitation Team supports adults living in the community with complex mental health issues.
Our services
We provide support that helps recovery including:
- living skills (self-care, community living skills, daily activities)
- links to resources in the community
- education on illness and how to manage symptoms.
The team
Your Mobile Intensive Rehabilitation Team will include nurses, social workers, occupational therapists and psychologists. You will continue to see your own mental health doctor and will be assigned a clinician to work with you individually.
How to access this service
The Mobile Intensive Rehabilitation Team provides services to consumers living within the mental health catchment area who have been referred by a mental health team.
What to bring
- Your Medicare card
- Health Care Card and/or concession card (if you have one)
- Recovery goals and plan
- Medications you are currently taking, including any that you have bought without a prescription and herbal medicine
- Private health insurance card (if you want to use it)
- Glasses, hearing aid, walking frame
- Your appointment letter
- Any special items listed on your letter
- Snacks, a drink or money to buy refreshments
