As a registered nurse for the Caboolture post-acute care service, Nicole travels as far as Kilcoy, Donnybrook, Bribie Island and Narangba visiting patients in the two weeks after a patient leaves hospital. 🚗

Nicole packs her car with dressings and medications based on her patient list for the day, plus extra supplies just in case, to treat patients needing help with home oxygen, medication and diabetes management, stoma and wound care, personal care as well as help with cognition decline and mobility related issues.

"Our service is wonderful in that it allows patients to go home with support in place for two weeks, ensuring they are safe to be at home," Nicole said. 🥰

The Caboolture Hospital Women, Children and Family Services centre located on Leda Boulevard in Morayfield, provides a space for antenatal and post-natal visits for women and their families who opt for a continuity of care model through midwifery group practice.

Robyn, who has 45 years of experience up her sleeve, is a clinical midwife and team leader of one of the midwifery group practices operating out of the centre - 'Midwife & Me', alongside Damara, Starting Well, and lactation consultants who run a clinic two days a week.

"The continuity of care model has proven to be a positive experience for mothers throughout their pregnancy, labour/birth and postpartum period," Robyn said.

"This space is dedicated to the care of women as it's primary role. I love the flexibility it gives us to see mums based on their individual needs, and in a relaxed atmosphere that is easier to access that feels less clinical."
