Hospital in the Home (HITH) Service

The Hospital in the Home service provides the same high standard of care as you would receive in hospital, but in the comfort of your home or another selected location.

Our team will contact you on a daily basis to provide treatment and monitor your progress. We will also develop an appropriate treatment plan to meet your care needs.

Our services

Our service offers a wide range of services from healthcare professionals including:

• doctors
• nursing staff
• occupational therapists
• physiotherapists
• social workers.

We also provide:

• help with scheduling and facilitating clinic reviews
• after hours telephone support
• ongoing discharge planning
• onward referrals.

Once your condition has improved and you no longer require hospital-level treatment, your care will be transferred back to your GP.

How to access this service

No external referrals are accepted for this service. You can only access this service when you are staying in hospital or if you have been referred by a hospital clinician.
