    The Healthy Ageing Assessment and Rehabilitation Team (HAART) is an interdisciplinary team comprised of a range of clinicians with a special interest in the rehabilitation and health of older people living in the community. The team includes a Rapid Occupational Therapist, Medical Specialist in Rehabilitation, Physiotherapists, Occupational Therapist, Speech Pathologist, Psychologist, Social Work, Dietetics and Pharmacy. Through a collaborative approach, our team is dedicated to providing innovative, patient focussed care that promotes health ageing. Our service is designed to Provide time-limited interdisciplinary assessment and management to prevent avoidable hospital presentations from age related medical conditions.

    Services

    The service provides comprehensive assessment, care planning and intervention for patients over the age of 65-years or older than 50 years for Aboriginal and Torres Strait Islander people.

    The service provides individual and group-based rehabilitation to improve wellbeing, functional capacity, independence and quality of life after a recent health event, functional decline or injury. Our service is designed to support continued independence in the community and optimise the health and wellbeing of our older population.

    What can you expect

    You will require a referral to attend this service.

    You will receive a phone call from our intake clinician who will ask some basic screening questions to help us understand your needs and goals. From here, you will be offered a face-to-face appointment for a comprehensive assessment where we will gather information about your past medical history, daily life and current concerns to help direct your care. This could be with any member of our team. You will then undergo assessments and therapy based on your concerns and goals with our team members.

    You will be expected to come into Kallangur Satellite Hospital for your visits and this could be up to two to three times per week. If you are unable to attend, it is expected that you call us on 07 3285 0000 to let us know. Our clinicians may provide 1-2 home or community visits if appropriate.

    Length of program

    You will be advised of your program length by the team and our team members will help coordinate any necessary follow up after you have finished with our program.

    How much does it cost?

    HAART is a completely subsidised program – there is no cost involved.

    Information for patients

    If attending the Rehab Gym for therapy, please wear comfortable clothing that is easy to move in, closed in shoes and bring a water bottle if you will be doing physical activity.

    You are welcome to bring a family member, carer or friend with you to your appointment.

    It is helpful to bring along a current medication list, any recent medical reports or My Aged Care Support Plans to your comprehensive assessment.

    How to refer

    To discuss a referral with our intake clinician, please call 07 3285 0071.

    You can submit a referral to our service via Smart Referrals. Alternatively, you can send a completed HAART referral form to KSH-OPARSIntake@health.qld.gov.au

    Why to refer

    Referral reasons may include:

    • falls prevention
    • pain management
    • communication or swallow impairments
    • cognitive impairments
    • management of changes to function related to progressive neurological conditions
    • mood impacting on activities of daily living
    • upper limb impairments and retraining
    • nutrition or dietetics concerns
    • medication advice
    • future planning or co-ordination of care.

    Eligibility criteria

    • Over 65 or Over 50 if an Aboriginal/Torres Strait Islander person.
    • Australian resident residing in the Kallangur Satellite Hospital catchment suburb:

      • Strathpine
    • Experiencing a health condition that impacts function and at risk of hospital presentation/admission due to age-related medical conditions.

    Patients will be ineligible if:

    • Currently accessing equivalent services in public or private sector.
    • Reside outside of catchment area.
    • Medically unstable requiring inpatient assessment or currently an inpatient.
    • Functionally stable and only require therapy for maintenance of chronic condition.
    • Requiring musculoskeletal physiotherapy treatment.
    • There are no rehabilitation goals.
