The Social Work Service provide a range of specialist and generalist services to assist patients, families and carers whose medical treatment is being provided through STARS. Social Workers assist people by helping to address psychosocial issues that may be impacting on their health.
Our services
We provide a range of specialised and generalist services to support you and your family while at STARS.
Our services include:
- Advisory role with difficult to solve practical problems.
- Working through difficult decisions that may be impacting on health.
- Assisting with psychosocial issues regarding recovery and return to normal activity post treatment.
- Crisis intervention.
- Developing and coordinating discharge plans from hospital appropriate to an individual’s needs.
- Grief counselling.
- Identifying appropriate resources and linkages within the community.
- Providing counselling to individuals and families.
- Providing information about the psychosocial impact of illness.
- Providing information and education about the health care system.
- Advance Care planning education.
- Assistance in transitioning to residential aged care, or accessing NDIS.
How to access this service
No external referrals are accepted for this service. You can only access this service when you are staying in hospital or if you have been referred by a STARS clinician.
What to bring
- Your Medicare card
- Private health insurance card (if you have one)
- Health Care Card and/or concession card (if you have one)
- Current medications (prescription, over the counter and herbal medicine)
- Relevant x-rays, scans or any other test results or reports
- Glasses, hearing and mobility aids
