Aged Care Assessment Team (ACAT)

We assess the medical, physical, cultural, psychological and social care needs of frail older persons to help them access a range of Australian Government-subsidised aged care services.

We provide aged care needs assessments to help older people access government funded support, including:

Queensland Health provides this service under the Australian Government’s Single Assessment System.

Who can use this service

Aged care needs assessments are for people over 65, and First Nations people over 50.

How to access this service

To use this service, ask your GP or health professional to send a referral through the My Aged Care website.

You can also call My Aged Care on 1800 200 422 or book an appointment online.

My Aged Care will talk to you to find out if you meet the criteria for an aged care assessment. If you’re eligible and they send us your referral, we’ll contact you.

Please tell us if you need any support during your assessment. This includes using an interpreter, talking with an Aboriginal and Torres Strait Islander liaison officer, or if you have any disability needs.
