Aged Care Assessment Team (ACAT)
We assess the medical, physical, cultural, psychological and social care needs of frail older persons to help them access a range of Australian Government-subsidised aged care services.
Our services
We provide aged care needs assessments to help older people access government funded support, including:
- support with daily living through the Support at Home and Commonwealth Home Support programs
- help recovering after illness, injury or decline through the Restorative Care Pathway
- short term care after a hospital stay through the Transition Care Program
- support to remain at home towards the end of your life through the End-of-Life Pathway
- permanent residential aged care and residential respite care.
Queensland Health provides this service under the Australian Government’s Single Assessment System.
