Community Based Rehabilitation Team (CBRT)
Community Based Rehabilitation Team (CBRT) aims to improve people’s well-being, independence and quality of life. People who will benefit from this service are those who have had a recent injury, illness or surgery (within the past six months) which has resulted in rapid functional decline and has impacted on their ability to complete everyday activities.
Our services
Our services and programs, including home visits (if required), run for 4, 8 and 12 weeks and include:
- Physiotherapy
- Occupational Therapy
- Speech Pathology
- Social Work
- Leisure Therapy
- Rehabilitation Consultancy.
We provide rehabilitation for people who are recovering from or have been diagnosed with:
- stroke
- acquired brain injury
- early stage or acute exacerbation of neurodegenerative conditions with rehabilitation capacity
- an orthopaedic condition
- lower limb amputation to provide prosthetic rehabilitation
How to access this service
To access this service, your GP or medical practitioner will need to send a referral letter.
If there is a waiting list, you will receive a confirmation letter and be advised on what to do next. If there is no waiting list we will contact you to arrange a suitable time for your appointment.

What to expect
You will require a referral to attend this service.
You will receive an initial appointment with one of our clinicians to do a comprehensive assessment to gather information about your medical history, daily life and current concerns to help us understand your needs and set your goals for rehabilitation. You will then undergo assessments and therapy based on your concerns and goals with our team members.
You will be expected to attend your appointments throughout your block of rehabilitation. This may be in the clinic, via Telehealth, or with a visit to your house. If you are unable to attend, it is expected that you call us to let us know.
Length of program
The blocks of therapy are usually 4, 8 or 12 weeks long, depending on your needs. This will be discussed at your comprehensive assessment.
How much does it cost?
CBRT is a completely subsidised program – there is no cost involved.
