Nurse Navigation

Nurse Navigation is a case coordination model of care comprised of experienced nurses with high level of clinical knowledge and understanding of the health care system that aim to help people with complex health care needs in navigating their health care journey from their general practitioner, hospital, specialist appointments and community supports.

Our services

  • Navigation team provides specialised clinicians in the areas of Disability, Frail and older person, Generalist and Aboriginal and Torres strait Islander.
  • Coordination of patient centred care: providing support, advocacy and care coordination across your health care journey, form community, GP, hospital and home again.
  • Creating partnerships: Work closely with your GP, medical specialists, allied health specialists, Aboriginal and Torres Strait Islander heatlhworkers, and other organisations (My aged care, NDIS) or care providers involved in your care to support you in your health care journey.
  • Improving patient outcomes: provide education to you and your family to support you to make informed decisions about your health care and advanced care planning, work closely with you and your family to develop health care goals and plans to assist you better manage your own health and promote independence, assist you navigate my aged care and NDIS or other support services needed so you can access the care support you need.

Service Scope

  • 18 years of age upwards or transitioning from paediatric services
  • Patients at risk of frequent hospital presentations
  • Poor health literacy
  • Carer stress and or poor community services engaged.
  • Multiple speciality outpatient appointments

Outside service scope

  • Home visit poses a risk to staff safety.
  • Patients whose primary issue is acute mental health – please refer to Mental Health Services
  • Patients whose primary issue is alcohol or drug dependence – please refer to ATODS.
  • Patient does not consent to referral to the Nurse Navigation Service
