Diabetes team provides diabetes education and care to patients staying at The Prince Charles Hospital.

Our services

We offer a range of educational services including:

  • diabetes education to adults with type 1 diabetes
  • introduction of insulin for type 2 diabetes
  • dietitian support.

Our team consists of doctors, nurses, dietitians, educators, social workers, exercise physiologists and psychologists.

What happens when I leave hospital?

After discharge, patients can be seen at one of our Specialist Diabetes Outpatient Clinics closest to their home, this includes:

  • Chermside Community Health Centre (adult diabetes)
  • North Lakes Health Precinct (paediatric diabetes and adult diabetes)
  • Caboolture Community Health Centre (paediatric diabetes and adult diabetes).

How to access this service

Inpatient referral only

No external referrals are accepted for the Inpatient Diabetes Service. You can only access this service when you are staying in hospital or if you have been referred by a hospital clinician.

GP or medical practitioner referral

To access the Specialist Diabetes Outpatient Clinic, your GP or medical practitioner will need to send a referral letter to Community, Indigenous and Subacute Services (CISS).

If there is a waiting list, you will receive a confirmation letter and be advised on what to do next. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time for your appointment.

What to bring

Each time you visit us
  • Your Medicare card
  • Private health insurance card (if you have one)
  • Health Care Card and/or concession card (if you have one)
  • Current medications (prescription, over the counter and herbal medicine)
  • Relevant x-rays, scans or any other test results or reports
  • Glasses, hearing aids and mobility aids
For an outpatient or clinic appointment
  • Your appointment letter
  • Any special items listed on your letter
  • WorkCover claim number (if relevant)
  • Snacks, a drink or money to buy refreshments
  • Something to read or do while you wait
