Community Geriatric Medicine

We provide a comprehensive geriatric medicine service to older people living in the community, at home or in residential aged care. This ensures specialist geriatric medical care is delivered to people who are unable to attend clinic appointments or prefer care in their own home.

Our services

We work with you, your family, carers and existing network of health professionals. Our services include:

  • home visits
  • comprehensive assessment of an older person’s physical, mental health and function
  • treatment planning
  • advice and education
  • referral to healthcare services within the community.

What to expect

One of our registrars will be in contact with you directly to make an first home visit appointment which may last between 1 and 2 hours. During your first appointment the registrar will discuss with you the need for any further appointments.

When we visit you

Please ask a family member, friend or carer to be with you when the registrar comes to visit as they can often provide very useful information about your health and wellbeing. Please ensure any dogs are suitably restrained.

How to access this service

To access this service, your GP or medical practitioner will need to send a referral letter to the hospital.

If there is a waiting list, you will receive a confirmation letter and be advised on what to do next. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time for your appointment.

What to bring

Each time you visit us
  • Your Medicare card
  • Private health insurance card (if you have one)
  • Health Care Card and/or concession card (if you have one)
  • Current medications (prescription, over the counter and herbal medicine)
  • Relevant x-rays, scans or any other test results or reports
  • Glasses, hearing aids and mobility aids
