New community service supports healthy ageing, hospital avoidance and early discharges

2022-06-27T10:18:12+10:0022 June 2022|COH Express|

New community service supports healthy ageing, hospital avoidance and early discharges. An enhanced community healthcare service will improve the care provided to the elderly, reduce hospital presentations and ensure faster patient discharges from hospital. Community and Oral Health Acting Nursing Director Vicky Sinclair said the Post-Acute Care Service (PACS) had introduced a geriatrician-led model of care that would enhance the care available to the elderly and support healthy ageing. “Demand for care that is accessible and convenient for our elderly patients in their home or in the community is increasing,” Vicky said.

Executive Director Message

2022-06-22T13:45:35+10:0013 June 2022|Community & Oral Health Directorate, COH Express|

The enhancement of our Post Acute Care Service is an important milestone for the directorate and reflective of the demand for accessible and compassionate care that supports the needs of our community. The introduction of a geriatrician-led model of care will enhance the care that Metro North Health delivers to the elderly and will support healthy ageing. This model of care wasn’t something that was implemented overnight and involved a lot of planning and behind the scenes work by many across our service.

I am inspired by the Community and Oral Health team’s ability to take action when required, to take on something new, meet tight time frames and to maintain and increase the commitment to service delivery.

A huge thank you for the tremendous service that you provide. Some of you may not actually appreciate what it means to a patient to have you attending to their needs daily. What you may consider a daily routine, is to me (and I am sure many others) a reminder of the good things in our community.

Former RBWH patient Jacinta Smith did just that recently, visiting RBWH to bring a photo collage showing her journey in the ICU to where she is today, as well as a hamper of delicious treats for the staff who brought her back to life.
