Here is the second Accreditation update to assist TPCH staff in preparing for short notice assessment which can occur at any time from July 1.

Today’s update will cover Standard 2 (Partnering with Consumers) and Standard 8 (Recognising and responding to acute deterioration).

Standard 2: Partnering with consumers

Intention of the standard

  • To create health service organisations where there are mutually beneficial outcomes by having:
    – consumers as partners in planning, design, delivery, measurement and evaluation of systems and services
    – patients as partners in their own care, to the extent that they choose.
  • To recognise the importance of involving patients in their own care and providing clear communication to patients
  • This standard, together with the Clinical Governance Standard, underpins all the other standards

TPCH priorities

  • Ensuring there is documented evidence in the patient record of shared decision making
  • Supporting our patients to provide feedback on their care
  • Ensuring consumers receive the information they need in a way that is appropriate for them
  • Ensuring Aboriginal and Torres Strait Islander people receive health care that meets their needs
  • Actioning the Metro North Policy that supports flexible visiting for our patients

What can you do?

Standard 8: Recognising and responding to acute deterioration

Intention of the standard

  • To ensure that a person’s acute deterioration is recognised promptly and appropriate action is taken. Acute deterioration includes physiological changes, as well as acute changes in cognition and mental state.

TPCH priorities

  • Improving mandatory training compliance with Basic Life Support (BLS) and Advanced Life Support (ALS)
  • Improved documentation of mental state observations
  • Supporting patients to better understand Ryan’s Rule

What can you do?

  • If you have previously had ALS allocated to you on TMS and no longer require it, please speak with your Line Manager to have it removed
  • Book in your BLS or ALS if it is overdue
  • With every set of observations documented on the Q-ADDS tool ensure you are assessing for changing behaviour or new confusion
  • Watch this video on Ryan’s Rule and support your patients to understand how to enact it if needed
  • If your patient wants written information on Ryan’s Rule, please ensure you have the most up to date brochure available in your area
