The Zillmere Residential Transition Care Program is for the older person who requires more time and support to complete their restorative care after a hospital stay.
The program is delivered in a non-hospital residential setting at Zillmere and supports people to improve their functional capacity and finalise and access their longer-term care arrangements.

- More than 80 per cent of our patients are aged 70 years or over
- 63 per cent of our patients are women
- Close to 40 per cent of patients return straight home after their admission
- Each year we care for more than 600 patients with an average length of stay being 34 days.
