A draft Metro North HHS Diabetes Clinical Service Plan 2021-2026 (the Plan) has been developed to guide diabetes service delivery across the HHS over the next five years. In scope for the Plan are diabetes services for persons aged 15 years and older associated with pre-diabetes/impaired glucose tolerance; Type 1 diabetes mellitus (T1DM); Type 2 diabetes mellitus (T2DM); and gestational diabetes mellitus (GDM).
The plan addresses:
• linkages with other specialty services that treat common diabetes complications
• service settings including hospitals, general practice, community-based care, primary care and the patient’s home
• the needs of Aboriginal and/or Torres Strait Islander persons and those from other culturally and linguistically diverse groups.
