Staff within the Home Hospital service will be aware of the Home Hospital Review that was undertaken earlier this year to provide advice and recommendations on how we can align programs of care with contemporary practices, prepare for future demand and ensure sustainability.

I am pleased to inform you that this review is now complete and we have received the Final Report.

I would like to thank all the staff members who have provided input into this review, as your input was invaluable in informing the review findings and recommendations.

Following receipt of the Final Report, a Home Hospital Reform Project group has been established to carefully consider all the recommendations that were made, and to undertake a program of work to review the services within our Home Hospital stream.

This project will be led by Mary Wheeldon and Dr Stephanie Hadikusumo and is likely to be completed by early 2024.

The first phase of this work is assessing whether the services within our COH Home Hospital have similarities and synergies in processes, relating to referrals, triaging, mode of service delivery, workforce profile and utilisation, education training, and outcomes monitoring.

The second phase will identify different options that may streamline these processes, that are responsive to consumer’s needs, and create efficiency and improved governance and service delivery.

Before any decisions are made, it is vitally important that all staff within our Home Hospital service stream have opportunity to understand the review findings, consider the recommendations, understand the impact of any proposed changes and provide feedback.

The first of these opportunities is on July 12, 2023 at 8.30 am when key members of our project team will be presenting the Review findings at a number of COH sites. These sites are detailed below.

There will be a Teams link (to the North Lakes site) and the presentation will also be recorded for those who are unable to attend.

There will be a further opportunity on July 19, 2023 at 8.30 am to meet with project team members and to bring up any additional concerns or questions.
