About North West Community Health Centre

Post Acute Care & PAC Wound Service North West - Jan - Dec 2022 statisticsHidden away in the suburb of Keperra, is a superb facility with great COH services and staff. 

The major service offered here are the Post-Acute Care Service (PACS) and wound clinic.

These vital community-based services provide short-term support for adults to avoid presentation or readmission to hospital.

Central to this care is patient appointments, care in the home and geriatrician care for the elderly.

The centre also houses other Metro North Health and Queensland Health services such as:

  • Child and Youth Community Health Services
  • Healthy Hearing Clinic
  • University Queensland Speech Clinic
  • BreastScreen Queensland
  • Brisbane City Council Immunisation Clinic.

FAST FACTS 1 (North West 2022)

1700 referrals were received last year from RBWH (62.1%) and TPCH (27.6%).

There were 12,000 occasions where our patients received great care and support in the home or at our clinics.

Staff Corner – The best thing about North West

North West Community Health Centre teamDuring a meeting with the Post-Acute Care Team, the executive shared staff acknowledgements and were left in no doubt that the team at North West enjoyed working as a team and loved the facility.

Earlier in the day, Kerri Gane, Senior Director and Kate Schultz, A/Director of Allied health observed the multidisciplinary team huddle and spoke highly of the teamwork, commitment to patient centred care and the focus on continuous improvement through the 15 Step process.

In the afternoon Allied Health leads Kate Schultz, Anita Keightley Assistant Director Allied Health and Kate Hacking Director Speech Pathology had the opportunity to discuss Allied Health workforce priorities with our dedicated PACs physiotherapists, social workers and clinical assistants.

Staff found the value of secondments and rotations across COH to provided opportunity to work in other areas a great incentive. Many staff are happy with the commute to work in such a great location.

The team told us that it’s the best place and spoke with pride about being a close-knit group, who all work as an effective team.

They are very comfortable discussing challenging issues as a multidisciplinary team.

We encourage Aspley Community Health Centre staff in particular to nominate a colleague or team for the February Shining Star Award by sending a 50-100 word submission to COH-Communications@health.qld.gov.au

Nominations will close on Friday 17 February.
